Family Outreach  

Outreach Worker

(Assistant Intake/Assessment)

(Approximately 28 hrs/week until March, 2009)

Program Name:  Family Outreach Program

Job Title: Outreach Worker with a focus on intake and assessment

 

Reports to: Family Outreach Supervisors

 

Job Summary:  Provide assessment of needs and determine referrals, provide outreach services to clients as well as support to other Outreach Workers on the team as required.

 

Key Duties & Responsibilities:

  • Interview, assess and outline services required.
  • Develop supportive interventions that will assist in determining goals and the means of attaining them.
  • Assist Outreach Workers facilitate groups required to manage a potential wait list
  • Act as an advocate and provides support for clients referred to Family Outreach.
  • Maintain contact with referred clients prior to Family Outreach Worker connection.
  • Respond to and track requests for support from the community.
  • Maintain accurate reports and records.
  • Maintain a thorough knowledge of community resources. 
  • Attend  mandated training as agreed upon with the supervisor;
  • Read, sign and follows the agency policies;
  • Perform other related duties as negotiated.

 

Qualifications & Experience:

  • A degree in social services or a related field - equivalent education and experience may be considered.
  • Minimum 3 years experience working with families and in a social services setting.
  • Must Maintain:
    • Clear criminal record
    • Emergency First Aid Certification
    • CPI (non-violent crisis intervention) Certification
    • Driver’s License
     

Job Skills and Abilities:

  • Superior communication and written skills in all areas.
  • Good organization, time and general management skills
  • Mediation Skills.
  • Ability to prioritize and determine client risks or needs
  • Ability to work effectively independently and with other program staff
  • Demonstrated professional approach to working with professionals and funders.
  • In-depth understanding of collaboration and integrated case-management.

 

This position requires staff to work in a stressful environment, dealing with clients in crisis situations.
